Cancellation/Refund Policy: You may cancel your BloomNow membership at any time through the “Account” section on the BloomNow portal (bloom24now.com/login) or by contacting us from the contact us page. 

Transfers of Membership: Membership is personal to you and may not be transferred to anyone else. 

Unpaid Balances: Members will not receive medication, testing, or other associated BloomNow services if they have unpaid balances. 

I agree to undergo laboratory testing (“the Test(s)”) from designated laboratories such as Quest Diagnostics, LabCorp. I understand these tests are solely for evaluating my health status. I acknowledge that I am seeking a review for weight loss medications by independent medical professionals. I understand that these licensed providers may decide to prescribe or not prescribe medication at their discretion. I recognize that BloomNow is a platform facilitating access to medical services and is not a healthcare provider. To receive the Test or Prescription, I may need to complete a questionnaire, and a physician or other authorized healthcare provider (“the Provider”) may evaluate my eligibility. I understand that part of my payment for services includes direct payments to independent medical professionals, for which BloomNow acts as a billing agent. These fees include a $10 fee for each lab or prescription request, a $20 fee for asynchronous lab result review and messaging, and a $35 fee for a synchronous telephone or virtual consultation. 

I understand that BloomNow supports the Laboratory and the Provider with non-medical services such as software and administrative services. BloomNow is not a clinical provider and will not provide healthcare or treatment. I release, waive, and forever discharge any claims against BloomNow related to the results of my Test or any information obtained during testing. 

I acknowledge and agree that: 

  • I am the individual providing the sample for the Test(s) I am requesting, or I am the parent or legal guardian of a minor providing the sample. 
  • I am at least eighteen (18) years of age, or I am the parent or legal guardian of a minor providing the sample. 
  • I have read and understand the information about the Test(s) and Prescription(s) provided to me. 
  • I am responsible for checking my email for results notifications and logging into my account to view my results when available. 
  • At the time of the Test, I will be physically present in the state I provided as my residence. I am responsible for forwarding any results to my primary care or other personal physician and for initiating follow-up care or treatment. 
  • I will not make medical decisions without consulting a healthcare provider or disregard medical advice from my healthcare provider based on the Test results. 
  • My test results will be reported to my state, local, and/or federal health agency as required by law. 
  • If I have questions before or after my Test, I can contact BloomNow at support@bloom24now.com. 

I consent to the Test and post-testing medical evaluation as outlined above. 

I consent to the BloomNow Membership Agreement as outlined above. By completing my intake, I understand my insurance will be billed by the lab. It is my responsibility to understand my insurance plan details, check if my insurance is in-network, or a preferred insurance of the lab of my choosing. I understand I can contact BloomNow at any time to change the lab. If I use my insurance, I may owe a copay and am responsible for any deductible. If I choose not to use my insurance, I will opt for out-of-pocket payment.
